CHNA Report Data

This 2016 Community Health Needs Assessment documents the findings from the 2012 Behavioral Risk Factor & Surveillance System (BRFSS), Hospital Discharge Data from 2011-2013, and Key Informant Interviews conducted as part of the assessment of needs in Middlesex County, and Somerset Counties.

Individual Characteristics
This chart displays our reporting pool demographics. The goal was to cast our net as wide as possible to encompass the broad array of citizens in our reporting area, allowing us to display accurate data regarding the people that make up our communities.
Percent Reporting Fair or Poor Self-Assessed Overall Health Status
The chart below shows the responses to the question, “Would you say that in general, your health is excellent, very good, good, fair or poor?” This is a widely-used measure for reliably predicting morbidity and mortality.
Percent Ever Diagnosed with Asthma
The chart below shows the combined county sample that reported ever being diagnosed with asthma (10.5%) compared to the New Jersey sample (12.5%).
Percent Reporting 4+ Days Mental Health Not Good Past 30 Days
The chart below shows the responses to the question, “Thinking about your mental health, which includes stress, depression, and problems with emotions, for how many days during the past 30 days was your mental health not good?”
Percent Ever Diagnosed with Diabetes
The chart below shows the combined county sample that reported ever being diagnosed with diabetes (8.9%) compared to the New Jersey sample (9.3%).
Percent Without a Regular Doctor
The chart below shows the combined county sample of respondents that reported that they did not have a personal doctor or health care provider (15.4%), compared to the New Jersey sample (17.4%).
Percent Who Could Not See a Doctor due to Cost in Past Year
The chart below shows the reported cost barriers in not accessing health care in the past year as reported in the combined county sample (12.4%) and in the state sample (14.9%).
Percent Current Smokers
The chart below shows the combined county sample of respondents that identified themselves as current smokers (12.6%) compared to the state sample (17.4%).
Percent Overweight and Obese
The chart below shows the combined county sample of respondents that identified themselves with height and weight measures classified as overweight and obesity (36.0% and 23.9%) compared to the state sample (36.9% and 24.6%).
Percent of Adults Who Have Never Had an HIV Test
The chart below provides a visual representation of the adults ages 18+ in the combined county sample that report never having had an HIV test (67.8%) as compared to the state sample (61.6%).
Avoidable Hospitalization Rate (per 100 population):
The chart below shows the rate of avoidable hospitalizations calculated for adults of age 18 or older the combined hospital service area sample (1.32/100 population) compared to the overall New Jersey rate (1.65/100 population).
Avoidable ED Visit Rate (per 100 population):
The chart below shows the rate of avoidable Emergency Department visits for all ages in the combined hospital service area sample (13.06/100 population) compared to the overall New Jersey rate (15.34/100 population).
Rates and Composition of Avoidable ED Visits (per 100 population):
The chart below shows overall rates of avoidable ED visits within the combined hospital service area as well as the type of visit based on classification schemes.
Payer Distribution of Avoidable Hospitalizations and ED Visits:
The chart below shows the payer distribution (Medicare, Medicaid, Private Pay, Self-Pay, or Other) of the avoidable hospitalizations and emergency department visits within the combined hospital service area.
Race-Ethnicity Distribution of Avoidable Hospitalizations and ED Visits:
