The 2019 Community Health Needs Assessment (CHNA) uses detailed primary and secondary public health data at the state, county, and community levels, from various sources including: the Department of Health and Human Services, Centers for Disease Control and Prevention, Census Bureau, Healthy People 2020, the County Health Rankings, hospital discharge data and Community Needs Surveys.
